Pathway Transformation Fund (PTF) 2025/26
The Pathway Transformation Fund (PTF) is part of the Health Innovation Network (HIN) Respiratory Transformation Programme (RTP) which is a two-year, nationally coordinated initiative launching in September 2025. It is led by Health Innovation Oxford and Thames Valley (HIOTV) in partnership with NHS England, the Office for Life Sciences (OLS), and the third sector, and aims to drive system-wide transformation in respiratory care across England.
This first wave of funding will support high-impact, integrated, evidence-based respiratory transformation projects between October 2025 and March 2026.
Funding priorities (both must be addressed):
- Early and accurate diagnosis to expand access to quality-assured spirometry and restoring diagnostic pathways.
- Risk stratification and optimisation to proactively identify and support rising-risk COPD patients with integrated, equitable care.
Funding overview:
- Total funding available: Up to £2.61 million (non-recurrent).
- Site allocation: Approximately 10 high-need systems; up to £300,000 each.
- Funding split: Approximately 60% for early diagnosis and 40% for risk stratification (±5% flexibility).
- Larger bids: Will only be considered in exceptional cases (e.g. regional collaborations or large population impact) and must include a strong value-for-money case.
Important notes on eligibility and submission:
- Applications must be submitted by a collaborative consortium of system partners, led by a single NHS lead applicant responsible for holding the contract, managing funds and coordinating delivery.
- Within any bid, the consortium must cover at least one locally defined neighbourhood footprint (see NHS England » Neighbourhood health guidelines 2025/26). To support broad geographic coverage, we ask each ICB to submit a single application. If you believe a second bid is warranted – for instance, due to an upcoming ICB merger or another unique configuration that will affect delivery between October 2025 and March 2026, please include a brief rationale in each submission.
- Early engagement with your local Health Innovation Network is strongly encouraged.
- Proposals must include sign-off from:
- An executive lead (e.g., Medical Director or CEO)
- The regional respiratory clinical lead (if in post)
- The ICB respiratory clinical lead (if in post)
